D-158 sets kindergarten registration schedule

ALGONQUIN – School District 158 will welcome new students for the 2013-14 school year during kindergarten registration Tuesday, Wednesday and Feb. 14 at the District Administration Office, 650 Academic Drive in Algonquin.

Children must be 5 years old on or before Sept. 1. Unless eligible for rights under the McKinney-Vento Act for homeless students, the following documents are required to enroll and register a student:

• Official birth certificate – a hospital certificate is not official. The needed birth certificate may be obtained from the county office in which the child was born.

• Proof of residency within District 158: Homeowners should bring a house closing statement or real-estate tax bill.

Renters should bring a current, signed lease agreement, proof of current month’s rent payment and a utility bill in the parent’s name (water, natural gas or electric) or a driver’s license with a current address.

• Two emergency contact names and phone numbers (other than parents).

• Proof of guardianship if the child is not living with birth parents.

Parents may drop off a completed Illinois Certificate of Physical Health Exam form, Vision Exam form and Dental Exam form.

To prevent overcrowding, the district asks parents with last names that begin with A through H to register from 9 a.m. to 12:30 p.m. or from 5 to 7 p.m. Tuesday.

Parents with last names that start with I to P may register from 9 a.m. to 12:30 p.m. or from 5 to 7 p.m. Wednesday.

Parents with last names that begin with Q to Z may register from 9 a.m. to 2:30 p.m. Feb. 14.

Those who cannot attend on the day listed are free to attend one of the other registration dates listed.

Those unable to attend any of the registrations should come to the District Registration Office after Feb. 25, but not later than April 1.

Parents should bring the kindergartener at the time of registration so the district may take an ID photo.
