Lizbeth A. Salazar, 26, of the 500 block of Somerset Lane, is charged with forgery, possession of prescription forms and obtaining a substance by fraud, according to charging documents.
The most serious charge, forgery, typically is punishable by three to five years in prison.
She is accused of using prescription forms with a Johnsburg doctor’s name on them from December 2016 through July and impersonating a Walgreens employee to call in refills, the charging documents show.
Johnsburg police were not available to comment on details, including where Salazar got the prescription forms.
In the eight-month period, Salazar used the false prescriptions to get painkillers and sedatives, charging documents show.
Salazar’s bond was set at $20,000.
She posted bail Wednesday afternoon and was released from the McHenry County Jail, according to the sheriff’s booking log.
Salazar is due in court Dec. 14.
