Advance medical directive program scheduled
CRYSTAL LAKE – The Partners Coalition for Improved End of Life Care will offer a program about advance medical directives at 7 p.m. Oct. 23 at the McHenry County Conference Center.
The focus of the workshop is to encourage people to complete a living will and a health care power of attorney. The importance of having a conversation with family and doctors about your wishes regarding health care decisions also will be emphasized.
The program will include a panel of health care providers, clergy and educators. Advance directive forms will be available.
The Partners Coalition is a nonprofit organization made up of health care providers, social-service agencies, long-term care facilities, faith communities, and educators. The Partners Coalition is affiliated with the National Hospice and Palliative Care Organization.
This program is free and open to the public.
